PHI is any individually identifiable health information that relates to:
- Your past, present, or future physical or mental health condition
- Healthcare services provided to you
- Payment for healthcare services
- Information that identifies you or could be used to identify you
How We Use and Disclose PHI
With Your Written Authorization
We may use and disclose your PHI with your written authorization for purposes including connecting you with treatment facilities, verifying insurance coverage, and coordinating care. You may revoke authorization at any time in writing.
Treatment, Payment, and Healthcare Operations
We may use and disclose PHI for:
- Treatment: Coordinating your care with treatment providers
- Payment: Verifying insurance benefits and processing payments
- Healthcare Operations: Quality improvement and business operations
Required by Law
We may disclose PHI when required by federal, state, or local law, including court orders, subpoenas, or public health reporting requirements.
Special Protections for Substance Abuse Records
Important: Information related to substance abuse treatment is subject to additional federal confidentiality protections under 42 CFR Part 2. We cannot disclose this information without your specific written consent, except in limited circumstances permitted by law.

You have the right to inspect and obtain a copy of your PHI maintained by us.
Right to Amend
You may request amendments to your PHI if you believe it is incorrect or incomplete.
Right to an Accounting of Disclosures
You may request a list of certain disclosures of your PHI we have made.
Right to Request Restrictions
You may request restrictions on certain uses and disclosures of your PHI.
Right to Confidential Communications
You may request to receive communications about your PHI by alternative means or locations.
Right to a Paper Copy
You have the right to obtain a paper copy of this notice upon request.

Breach Notification

We may share PHI with business associates (such as treatment facilities, insurance verification services, or IT providers) who perform services on our behalf. All business associates sign agreements requiring them to protect PHI and comply with HIPAA regulations.
Filing a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

Changes to This Notice
We reserve the right to change this notice and make the new provisions effective for all PHI we maintain. Current versions will be posted on our website with the effective date.
